THE SERVICE PROVISION Exclusive Care Ltd offers a range of supportive options for service users who have been identified as having needs that are complex in terms of support, housing, enablement and community participation. Options include: • Support in the home with daily-living tasks and personal development. • Support to assist the individual with activities and social inclusion. • Support for college, skill training and employment. • Support to allow the individual to have a holiday in a location of their choice. • Support in the home to manage a tenancy agreement to enable community integration. I. SUPPORTED LIVING: Supported living and domiciliary care is based on the principal that disabled people should be able to live in their own home, with appropriate flexible support, whatever their needs. Using standard residential housing units provided by local authorities, housing associations or mainstream rented housing, individuals can be supported by a “living support network”. Exclusive Care places great emphasis on the positive aspects of supported living as a tool to facilitate positive development. Sometimes a service user will choose to live separate from other service users while other clients will choose to live with a client they have developed a friendship with. If a service users chooses to live on their own then the positive inclusion develops the day to day relationships available to the rest of society. Individuals are assisted as part of their person centred planning process to find properties which are within established community facilities. The properties are adapted and equipped to meet the needs of individual tenants and are maintained, decorated and furnished to a high standard. Support is offered around the personalisation of people’s homes, where they can use their own furniture, fittings and personal effects. A secure tenancy, or licensing agreement, is essential to the maximisation of independence.
